Steps to Obtain your Medical Marijuana Patient Card
1st – Schedule an Appointment With A MMJ Doctor Or Clinic
2nd – Obtain Your Medical Records
In Arizona, medical marijuana is approved for the following conditions:
- Severe & Chronic Pain
- Severe Nausea/Vomiting
- Severe or Persistent Muscle Spasms
- Wasting Syndrome •Epilepsy •Hepatitis C •Seizures
- Glaucoma •Crohn’s Disease •Alzheimer’s •HIV/Aids
- Cancer •Cachexia •Multiple Sclerosis •ALS / Lou Gehrig’s
You may ask your current physician to provide your records, or you may ask either of these certification centers to recommend an alternative doctor to provide your medical records.
3rd – Meet with a Physician at the Certification Center
Once you have obtained your medical records, you will meet with a physician at the certification center that will make a recommendation that you obtain your Medical Marijuana Registry Card. The Certification Center will complete and file all of the necessary paperwork with the state at no additional charge. You will receive your Arizona Medical Registry Card from the State of Arizona Health Department within 10 days.
